65+ & 2+ Comorbidity Vaccine Waitlist

East Longmeadow is moving forward in the vaccination process as quickly as possible. We understand all of our residents are anxious to be vaccinated. 

While Phase 2 Step 2 is not officially eligible for vaccines at this time, we want to be prepared as possible when that phase does open, so we are opening a wait list for those 65 and over, and those with 2 or more Comorbidities. You can be added to the waitlist by one of the following ways: Complete the google form HERE, or EMAIL your name, age, address and phone number to vaccine@eastlongmeadowma.gov . By adding your name to the waitlist, there is no timeline associated with this process. Our goal is to be as prepared as possible once the next phase opens. 

The CDC lists ONLY the following conditions as comorbidities. 
The Commonwealth has launched an online attestation form. If an individual presents a signed paper or electronic copy of the attestation form that indicates they are in an eligible category to receive the vaccine, then the individual has sufficiently demonstrated their eligibility. 

Please know, our vaccination efforts are greatly restricted due to the amount of vaccines we are receiving. The State is providing us with 100 vaccines a week. You are putting your name on a waitlist for the vaccine. We will not move into Phase 2 Step 2 until we have completed all our residents who are over 75 and on our waitlist. 

Again, It is very important to note we will never try to sell you anything, ask for you for your social security information, credit card or bank info or ask you to transfer money. This is a free vaccine. When you are contacted to register we will request your name, date of birth, address, medical history, and insurance information. If you have any concerns about the legitimacy of the call please hang up and call 413-525-5440 and the police department will assist you.
